The answer reveals more about quality than anything else, because it is precisely these steps that determine whether the result looks natural.

The two models

There are broadly two models. In the doctor led clinic an experienced specialist designs and performs the critical steps, while a trained team assists. In the technician led clinic, often a 'mill', technicians perform most of the procedure with minimal medical involvement, sometimes on several patients at once. The doctor may only be seen briefly at the start.

Doctor led clinic: Higher quality in the critical steps, better naturalness and graft survival, more individual planning, but typically a higher price and a longer wait. Technician led clinic: A lower price and quick capacity, but at the expense of care, with a greater risk of unnatural results, low survival and an exhausted donor. For most patients the doctor led model is clearly preferable, precisely because a poor result is permanent and expensive to correct.

What you should insist on

You should insist on a clear account of who does what on the day of the operation: who administers the anaesthetic, who designs the hairline, who opens the channels, and who places the grafts. It is entirely legitimate to ask, and a serious clinic has nothing to hide. Ask also to meet the surgeon who will operate on you, rather than only a patient coordinator. The way a clinic responds to these questions, openly and concretely, or evasively and in generalities, is in itself one of the clearest signs of quality you can get, even before you have made up your mind.
