The first weeks are critical for graft survival, and both smoking and alcohol get in the way of healing.
Why the first weeks are critical
During the first weeks the newly placed grafts have not established their own blood supply. They are entirely dependent on the surrounding tissue delivering oxygen and nutrition while they anchor themselves. Anything that impairs blood flow in the scalp during this phase directly costs graft survival, and therefore your result.
That is why smoking and alcohol are not merely "unhealthy" in the ordinary sense here; they strike at exactly the mechanism that determines whether the follicles survive.
How smoking does harm
Nicotine is a vasoconstrictor: it contracts the blood vessels and reduces the oxygen and blood supply to the vulnerable grafts, just when they need it most. At the same time cigarette smoke contains carbon monoxide, which further lowers the oxygen capacity of the blood.
The result is poorer healing, an increased risk of graft loss and slower wound closure. Smoking can also worsen scarring in the donor area. Vaping and nicotine replacement count too: it is the nicotine, not only the smoke, that is the problem.
How alcohol does harm
Alcohol acts as a blood thinner, which can increase bleeding and swelling in the first few days. It also causes dehydration, which does not help healing, and can interact with the medication you have been given, as painkillers and antibiotics should not be mixed with alcohol.
On top of that, alcohol dulls your judgement, and you become less careful about protecting your head, sleeping correctly and following the washing protocol.

The recommendation
Most clinics recommend:
- Smoking: avoid for at least 1-2 weeks before and 2 weeks after, and the longer the better
- Alcohol: avoid for the first 5-7 days, and particularly while you are taking medication
Always follow your clinic's specific protocol, as it may differ. Taking a break during this short, critical period is a cheap investment in a result you have to live with for the rest of your life.

Smoking and alcohol are only two of the factors that affect healing. Sweat, sun, hard training and incorrect washing also play a part in the critical phase. Thinking of the grafts as "delicate newly planted seedlings" for the first couple of weeks helps you make the right choices.

What about caffeine, diet and medication?
Smoking and alcohol are the most obvious culprits, but they are not the only factors in the critical phase. Strong caffeine immediately after the procedure can in some people raise blood pressure and restlessness, so enjoy it in moderation. Blood thinning medication and certain supplements (for example fish oil, high doses of vitamin E) should only be resumed by agreement with your doctor, as they can increase bleeding.
